Photography/Video Consent (including Zoom/Google Meet sessions and similar)

I herby give consent for photography/Videos and audio recordings featuring me to be used by CRAZY STRENGTH UK LTD, which may be used for social media, marketing, movement analysis or any other lawful basis.

Personal information consent under GDPR:

I herby give consent for my personal information to be securely recorded in a GDPR compliant Accident or near Miss Book should an incident occur.

CCTV

I am aware that CCTV in being used at this location for Health & Safety/Crime reasons and that my activities may be recorded.

Express assumption of risk

I, the undersigned, am aware that there are significant risks involved in all aspects of physical training. These risks include, but are not limited to: slips/trips/falls which can result in serious injury or death; illness, injury or death due to negligence on the part of myself, my training partner, or other people around me; illness, injury or death due to improper use or failure of equipment; strains and sprains. I am aware that any of these above-mentioned risks may result in illness, serious injury or death to myself and or my partner(s). I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any illness, injury or death that may result from participation in any activity or class while at, or under direction of CRAZY STRENGTH UK LTD that is not due to a failing in Duty of Care by CRAZY STRENGTH UK LTD. I acknowledge that I have no physical impairments, injuries, or illnesses that will endanger me or others.

Personal Property

I hereby acknowledge that CRAZY STRENGTH UK LTD has no responsibility for personal possessions. Any equipment or possession that is not owned by CRAZY STRENGTH UK LTD is the sole responsibility of the individual from a liability and indemnity perspective.
Release

In consideration of the above mentioned risks and hazards and in consideration of the fact that I am willingly and ,voluntarily participating in the activities offered by CRAZY STRENGTH UK LTD, I, the undersigned hereby release CRAZY STRENGTH UK LTD, their principals, agents, employees, and volunteers from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my participation in this activity, including those allegedly attributed to the negligent acts or omissions of the above mentioned parties. This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect.

If I am signing on behalf of a minor child, I also give full permission for any person connected with CRAZY STRENGTH UK LTD to administer first aid deemed necessary, and in case of serious illness or injury, I give permission to call for medical and or surgical care for the child and to transport the child to a medical facility deemed necessary for the well being of the child.

Abuse of equipment or area

I agree to adhere to the cleanliness and equipment care mandated by CRAZY STRENGTH UK LTD. All equipment should be cleaned and replaced from the place it is stored. Chalk is to be brushed from bars and any spillage cleaned by prior to departure. No bars are to be dropped unless they have AT LEAST 10kg bumper plates either end. Any non-conformance of any of the above will lead to the instantaneous ceasing of the current session OR in extreme circumstances, cancellation of your membership without notice.

Indemnification

The participant recognises that there is risk involved in the types of activities offered by CRAZY STRENGTH UK LTD. Therefore the participant accepts financial responsibility for any injury/illness that the participant may cause either to him/herself or to any other participant due to his/her negligence. Should the above mentioned parties, or anyone acting on their behalf, be required to incur legal fees and costs to enforce this agreement, the participant agrees to reimburse them for such fees and costs. The participant further agrees to indemnify and hold harmless CRAZY STRENGTH UK LTD, their principals, agents, employees, and volunteers from liability for the illness, injury or death of any person(s) and damage to property that may result from their negligent or intentional act or omission while participating in activities offered by CRAZY STRENGTH UK LTD, at the gym itself or any other location. This includes but is not limited to training abroad, or at parks, recreational areas, playgrounds, areas adjacent to the gym, and/or any area selected for training by CRAZY STRENGTH UK LTD

Online/Virtual training (if applicable)
The participant accepts all responsibility for attending the virtual session run by CRAZY STRENGTH UK LTD on time, and in a ready state to undertake physical exercise, including but not limited to: athlete capability and use of scaling options, equipment safety and maintenance, safe and suitable training spaces, surfaces and clothing. Athletes under 18 must attend specific sessions for minors and have the permission of an adult. Supervision is also recommended. By undertaking sessions, the participant also takes responsibility for being in an adequate fit and healthy state or otherwise having medical permission to exercise. The participant also agrees to stop the training if there are any health & safety concerns and to review with CRAZY STRENGTH UK LTD directly after the session has finished to discuss.

I have read and understood the foregoing assumption of risk, and release of liability and I understand that by signing it obligates me to indemnify the parties named for any liability for illness, injury or death of any person and damage to property caused by my negligent or intentional act or omission. I understand that by signing this form I am waiving valuable legal rights.

CANCELLATIONS & FEES

Late cancellations without a minimum notice period could incur a charge. All ‘no shows’ will be subject to a charge of £1. All fees will be charged to the card registered to the account, any profit made from these charges will be donated to the Clubs chosen charity

CANCELLING YOUR MEMBERSHIP

You can cancel your membership at any time by giving us at least 30 days notice ahead of your next direct debit date. You will need contact us direct informing us in writing (E-mail) via contact@crazystrength.co.uk. If you do not give us at least 30 days notice ahead of your next direct debit date, you will be charged as normal on your next direct debit date and your membership will cease at the end of that month.

FREEZING OF MEMBERSHIP

You are permitted to freeze your membership once per calendar year for up to 30 days in the case of injury or extenuating circumstances.

SOUTHERN QUARTER FACILITY RULES

The minimum age for use of the Facility and classes unless specified is 16. People under the age of 16 are required to provide a certificate from a parent or legal guardian confirming approval to train at the Facility.

Facility Rules apply to everyone attending the Facility or participating in a class. Facility Rules form part of this agreement so you must make sure you read, understand and follow them at all times. If you break any of the Facility Rules, we will respond in a way we consider fair and appropriate. For example, in less serious cases, we may give you a warning but in serious cases or where you have repeatedly broken Facility Rules we may suspend or cancel your Membership. If your breach causes us or another person costs, loss or damages, you agree to pay for these.

You promise to take care to use the facility and all exercise equipment safely and properly. If you are ever not sure how to operate any equipment properly, you must ask an Instructor or another staff member before you use it.
You agree to pay for any loss or damage to the facility and its equipment caused by you through a wilful, wrongful or negligent act or as a result of your breach of this Agreement.

You agree to follow any reasonable direction of a member of facility staff relating to health, safety or security or related matters.
You promise not to unnecessarily bring valuables into a facility and agree that it is not the obligation of the Facility to look after unattended property.
Your personal items are your responsibility. We will not be held liable for any loss, damage or theft of any personal item on our premises.

ACCIDENTS OR INCIDENTS

Accidents or Incidents during class timings are to be reported to the coach responsible for that class and recorded in the accident/incident folder. Failure to do so will relinquish CRAZY STRENGTH UK LTD of any responsibility or the accident/incident. In the instance of Cub/Under Age/Minor athletes ANY suspicious activity or changes in behaviour that are resultant from interactions at CRAZY STRENGTH UK LTD are to be fed to Performance Director Craig Spicer and recorded in the Cubs Incident log immediately.

INTELLECTUAL PROPERTY RIGHTS (IPR)

The use of the clubs branding/logos and any associating identification is the intellectual property of CRAZY STRENGTH UK LTD. Permission is to be sort prior to the use of any of CRAZY STRENGTH UK LTD IPR. Lawful proceedings will be sort for anyone found not adhering to this.

CRAZY STRENGTH FACILITY RULES

The equipment is to be used with care. No barbells are to be dropped from any height unless they have AT LEAST 10kg bumper plates either end. Any non-conformance of any of the above will lead to the instantaneous ceasing of the current session OR in extreme circumstances, cancellation of your membership without notice.
Upon completion of any session or class all equipment is to be cleaned, chalk removed and replaced to where it is stored.

Be respectful/mindful of other users. Not everyone likes to be shouted at, though if they do it is encouraged and refrain from standing in someone’s eyeline when executing a lift.

Special care is to be taken when operating around the windowed area of Crazy Strength. If irrational behaviour leads to the damage of the property you may be liable to pay for reasonable restoration of the damaged caused.

Please note that the Crazy Strength Open Gym only gives you access to the Crazy Strength area and equipment AND NOT to Southern Quarter Crossfit or BJJ Dojo.
